  • When does my calendar year start?
  • How many sessions per calendar year does my plan cover?
  • Does my plan have out of network benefits?
  • How much does my plan cover for out of network provider?
  • Is approval required from my primary care physician?
  • What does the company allow (“reasonable and customary”) for the following codes: 90791, 90832, 90834, and 90837.
  • Are there standardized forms I will need to submit for reimbursement? Where do I find them?

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.

Cancellation Policy

We require a 24-hour cancellation notification. If you miss your appointment without providing 24-hour notice, you will be assessed a full session fee for the missed appointment.

Phone Call Policy

If you would like to speak with me in between our scheduled sessions, you may call me. I will do my best to return your call within 48 hours during business days. Be aware that calls longer than 10 minutes will be treated as a therapy session and will be charged at a prorated rate of my fee every 15 minutes. If phone calls become frequent between session (e.g. more than once every other week), I will suggest that we address these issues during more frequent sessions so that I may better address them.
